copies of the following certificate self-attested should be enclosed along-with
the application form for All India Level Common Entrance Exam: MBBS Degree
Certificate, Provisional Pass Certificate I & II, Marks sheets of the
University Exam for the whole of the MBBS Courses, Attempt Certificate (First
year to Final year wherever applicable), Community Certificate,
Internship Certificate (C.R.R.I.), Permanent Registration Certificate issued by
Medical Council of India or respective State Medical Council, Transfer
Certificate, Migration Certificate (wherever applicable) and Conduct
Certificate issued by the Head of the Institution last studied
